Permanency Specialist

Spaulding Academy & Family Services is pleased to announce the addition of a Permanency Specialist to our Behavioral Programming team.

The Permanency Specialist provides customer service and case management services to families, children, and youth served by Spaulding. We value and support the role of families in their youth’s treatment and ensure the family voice is supported in their child’s treatment.  Permanency Specialists will need to be able to work in a flexible way and be able to multitask as needed for the needs of the parents and the rest of the team.  We believe in a “growth set model” where growth of a case is supported by a team of staff.  The role of Permanency is to navigate the youth’s treatment from admission to post discharge.  To enhance transition and after-care services by supporting families and youth through an individualized community-based, trauma informed, culturally sensitive supports during the onset of admission, throughout placement and for at least 6 months post discharge as indicated in order for which builds on an extension of the treatment and promotes safety and well-being by identifying and securing resources for the individual/ family in order to maintain the child in a family. To enhance and support incident debriefing with students and their families to be inclusive of family voice. Assisting, enhancing, and supporting families through the guardianship process.

PRINCIPAL DUTIES OF THIS ROLE INCLUDES:

  • Provide families weekly face to face support and education using a multi-systemic approach, or at frequency defined in treatment the plan
  • Coordinate quarterly treatment team meetings, as appropriate
  • Identify community resources, programs, and support networks on behalf of children and their families
  • Coordinate, document, and assist with transportation and family visit arrangements as defined by the treatment plan
  • Work closely with other assigned staff in providing case management for designated cases and work collaboratively with our Behavioral Health Team members to ensure coordination of care during the transition out of care and throughout 6 month after-care.
  • Serve as liaison by coordinating internal communication between other departments at Spaulding as well as external agencies (courts, schools, DCYF, mental health, etc.), and families through timely, effective, and professional communication
  • Complete all paperwork within the timelines expected (court reports, treatment plans, case notes, phone notes, monthlies, discharge reports, other documents as assigned) in compliance with expectations or Medicaid compliance and any other deadlines
  • Ensure youth involvement and engagement in discharge/transition process to include reviewing the individual rights in the process
  • Develop safety/crisis planning for the individual/ family or facility for all areas of programming through transition, discharge and after-care in conjunction with the other team members.
  • Assist, develop and coordinate guardianship components for individual/families, if applicable
  • Communicate and coordinate debriefing for all critical incidents on campus with the families.
  • Ensure after-care responsibilities are met for all cases to include documentation of all aftercare activities and reports
  • Participate in various meetings on and off grounds to include team meetings, court hearings, FAIR meetings, and matching meetings, permanency plan meeting, wrap around meetings, and staff meetings; communicate the pertinent details of each to appropriate parties.
  • Attend and participate in weekly group supervision lead by the supervisor and individual supervision 
  • Monitor, screen and respond to all pertinent communication regarding case assignments in a timely fashion.
  • Participate as a member of Resource on-call and perform related duties

EDUCATION, EXPERIENCE AND ABILITIES:

  • Minimally a Bachelor’s degree in Psychology or a related field
  • Experience in applied behavior analysis, support services and/or advocacy is desirable
  • This position requires experience in understanding children and family systems and behavioral approaches to treatment                                                                           
  • Strong knowledge and judgment
  • Organization skills
  • Excellent interpersonal and communication skills.  The person will have good oral and written language skills as well as good computer skills.  
  • Become trained in the use of PCM and Spaulding’ Behavior Management system (offered at Spaulding)

SCHEDULE: This is a full-time position where the hours are flexible in that it meets the needs of the student and the family.  Any evening or weekend work will meet the needs of the youth or their family.
