Admissions & Referrals
Spaulding Academy & Family Services accepts admission referrals from New Hampshire School Districts and state social service agencies from NH and beyond. During the 2016-2017 school year, 155 children and youth from 98 communities received support from one of Spaulding Academy & Family Services’ programs and services.
The Spaulding Academy & Family Services admissions process is both thorough and timely. Each stage of the process is designed to be informative and inclusive for the referral agency, parents or legal guardians, and the prospective student. In addition, each stage provides the necessary information to determine if Spaulding Academy & Family Services programs and services can meet the needs of the prospective student.
Referral
Applications for Spaulding Academy & Family Services’ programs and services are usually initiated by School Administrative Units (SAUs), New Hampshire Division for Children, Youth and Families (DCYF) or Juvenile Justice Services (JJS). Social services agencies from other states are also welcome to submit applications for consideration.
During this stage, the referring agency’s representative will contact Spaulding Academy & Family Services’ Admissions Director to discuss the needs of your child. If your child’s needs can be met by Spaulding Academy & Family Services’ existing programs and services and availability exists to provide the required support, then the referring agency will submit an application for thorough review.
Application
As part of the application stage, a complete background of your child is required for review by the Spaulding Academy & Family Services Admissions Team. This background information includes but is not limited to:
- Family history and chronological history of agency contacts.
- Discharge details and other reports from previous programs.
- Current Individual Education Plan (IEP), educational history and assessments
- Functional Behavioral Assessment and the Behavior Intervention Plan (FBA BIP), etc.
- All psychological and psycho-educational test results, including IQ scores.
- A complete medical history, including immunization records, medical insurance information and medication history.
- Court and probation records, if applicable, including most recent court report.
- Detailed, specific description of current difficulties and reason(s) for referral.
Upon receipt of this information, the Spaulding Academy & Family Services Admissions Team will review and confirm whether your child is a potential match for available programs and services.
