Current employees
How to retrieve your documents:
- You may access Paycor via a web browser or mobile app at any time to download an electronic copy of your W-2.
- Under your profile, please select the drop down for Pay & Taxes, then Pay Stubs & Tax Docs
- The needed tax year documents will be under the Tax Documents tab.
- In addition, a paper copy will be mailed to the address you have on file, postmarked no later than January 31st following the current tax year.

How to Request your documents
- Access to Paycor has ended once you have separated from Spaulding Academy & Family Services; To request your current W-2 or prior years W-2’s please fill out the request form below and a copy of your documents will be mailed to the physical address noted on the form within 1-3 business days.
- For electronic copies via email – Once a response to a verification email has occurred we will provide a copy within 1-3 business days.
- If you need to update your address or phone number with Spaulding Academy & Family Services, please fill out the form below so the changes can be made.
IRS Tax Form 1095-C (ACA) information
The Affordable Care Act (ACA) requires Spaulding Academy & Family Services to provide Form 1095-C to individuals who were offered or enrolled in minimum essential coverage. These forms provide information about the health coverage offered to you each tax year. Due to the Paperwork Burden Reduction Act we are no longer required to automatically mail these forms to all current and former employees and other covered individuals.
How to request your form
The Form 1095-C is available to you upon request, free of charge. To request your form, complete the form below, call, email or mail a written request as indicated below. Please note: Upon receipt of your request, we will furnish the form to you within 30 days, or by January 31 of the following year, whichever is later.

- You are not required to file Form 1095-C with your personal tax return. The form is for your records and provides information about the health coverage offered by your employer as required by the ACA.
- Current employees only: Your ACA form is available through Paycor.
